Patient Medical History
Please download our new patient form to fill in all your details and bring this form with you to your appointment.
Any information left unanswered will be requested and discussed during your appointment. All information that you provide to us is strictly confidential.
Our practice follows the guidelines as set out by the Australian Dental Association (ADA) and as such you will be asked to update your medical history each year. This is necessary to ensure we can help provide you with the best oral health care possible.
Cancellations And Changes
Please note: Tewantin Dental Centre requires 48 hours’ notice should you wish to change or cancel your appointment. Failure to allow the required notice may incur a charge.
Your First Visit
For your first appointment, leave plenty of time to locate our practice, we ask you to come in 15 minutes prior to your first appointment. This gives the team enough time to add your information into the system to brief your dentist or hygienist.
Once you have had your first comprehensive examination with us, you will be provided with an estimated cost for any treatment deemed necessary by the practitioner. This estimate is valid for six months from the date received.
Tewantin Dental Centre is equipped with HICAPS to enable you to claim directly from your private health fund on the day of treatment, so please remember to bring your card with you. Unfortunately, we’re unable to process claims without a card being presented.
Payment On The Day
Tewantin Dental Centre requests full payment at the time of your appointment. In some cases of complex treatment, we can provide payment plans to help avoid any delay in you receiving the treatment you need. For more information on payment plans, our reception team can assist. We accept Master Card, Visa, American Express, Cheques and Cash.
Parking on the street at the front of the building is available, but there is also parking at the rear of the premises via MOORINDIL STREET. These spaces are marked for DENTIST.
